5. Telemedicine Consent & Acknowledgment
By scheduling or participating in telemedicine services with Affinity Functional Health, you acknowledge and consent to the following:
a. Nature of Telemedicine
Telemedicine involves the use of electronic communications (video, phone, secure messaging) to provide healthcare services remotely. Telemedicine may include consultation, evaluation, diagnosis, treatment planning, and prescription of medications when appropriate.
b. Limitations of Telemedicine
