Services provided by Affinity Functional Health are personalized, wellness-oriented, and preventive in nature and are designed to support overall health through individualized evaluation, education, and care planning.

Nothing on this website or provided during visits should be interpreted as:

  • Emergency medical care

  • Acute care services

  • A guarantee of results

  • A replacement for a patient’s primary care provider or specialist when indicated

Patients should seek immediate medical attention or call 911 for medical emergencies, acute symptoms, or rapidly worsening conditions.

2. Scope of Care

Affinity Functional Health provides:

  • Functional and integrative health evaluations

  • Weight management and metabolic support

  • Telemedicine consultations

  • Personalized wellness and lifestyle guidance

  • Prescription medications when clinically appropriate and legally permitted

Affinity Functional Health does not provide:

  • Emergency services

  • Hospital-based care

  • Ongoing management of unstable or acute medical conditions

Care is provided only after appropriate intake, informed consent, and clinical determination of eligibility.

3. No Guarantees or Medical Claims

Health outcomes vary from person to person. Individual responses to wellness interventions, medications, supplements, dietary changes, or lifestyle recommendations cannot be guaranteed.

Any references to “optimization,” “support,” or “improvement” refer to potential benefits based on individualized assessment and do not represent guaranteed results.

4. Cash-Based Practice & Insurance Disclaimer

Affinity Functional Health operates as a cash-based practice.

  • We do not bill insurance for services provided under Affinity Functional Health.

  • Services rendered are not submitted to insurance for reimbursement.

  • Patients are responsible for payment at the time services are rendered unless otherwise stated in writing.

Patients enrolled in insurance plans acknowledge that participation in Affinity Functional Health services is voluntary and separate from insurance-based medical care.

5. Telemedicine Consent & Acknowledgment

By scheduling or participating in telemedicine services with Affinity Functional Health, you acknowledge and consent to the following:

a. Nature of Telemedicine

Telemedicine involves the use of electronic communications (video, phone, secure messaging) to provide healthcare services remotely. Telemedicine may include consultation, evaluation, diagnosis, treatment planning, and prescription of medications when appropriate.

b. Limitations of Telemedicine

You understand that:

  • Telemedicine has limitations compared to in-person evaluations

  • Certain conditions may require in-person evaluation, laboratory testing, imaging, or referral

  • Technical failures may occur despite reasonable safeguards

If the provider determines telemedicine is not clinically appropriate, an in-person evaluation or referral may be required.

c. Emergency Situations

Telemedicine is not appropriate for emergencies. If you are experiencing chest pain, shortness of breath, severe abdominal pain, neurological symptoms, or other urgent conditions, you agree to seek immediate local emergency care.

6. Prescribing & Medication Policies

Prescription medications, including those used for weight management and metabolic support, are prescribed only after:

  • A clinical evaluation

  • Review of medical history

  • Determination of medical appropriateness

  • Signed informed consent

Not all patients are candidates for prescription therapy. Decisions are made on a case-by-case clinical basis and may change over time.

Affinity Functional Health reserves the right to:

  • Modify treatment plans

  • Require follow-up visits

  • Discontinue medications if clinically indicated

7. Privacy & Confidentiality

Affinity Functional Health is committed to protecting patient privacy.

  • Telemedicine services are provided through HIPAA-compliant platforms

  • Patient health information is handled in accordance with applicable privacy laws

  • Patients acknowledge that no electronic system can be guaranteed 100% secure

A separate Privacy Policy outlines patient rights and data practices.

8. Licensure & Geographic Limitations

Providers with Affinity Functional Health are licensed to practice in the State of Texas.

Telemedicine services are offered only to patients physically located in jurisdictions where the provider is licensed at the time of the visit. Patients are responsible for confirming their location during telemedicine encounters.

9. Patient Responsibilities

By engaging in services with Affinity Functional Health, you agree to:

  • Provide accurate and complete health information

  • Follow agreed-upon care plans or communicate concerns promptly

  • Participate honestly in telemedicine encounters

  • Accept responsibility for healthcare decisions made collaboratively with your provider

10. Acknowledgment & Consent

By using this website, scheduling services, or participating in care with Affinity Functional Health, you acknowledge that you have:

  • Read and understood this disclaimer and consent

  • Had an opportunity to ask questions

  • Voluntarily consented to services under these terms
